AUTHENTICATION OF TELEPHONE ORDERS UPDATE


The Physician Champion model of authenticating telephone orders provided an avenue for the BHS medical staff to comply with the timely and complete authentication of telephone orders. The BHS is now transitioning from this model and engaging the medical staff members to authenticate their telephone orders within 96 hours.

As a reminder CMS, TJC and state law requires verbal/telephone orders to be legibly Signed, Dated and Timed within 96 hours.

If you are rounding on a colleague’s patient being treated under previous telephone orders, signing dating and timing these orders will be necessary to avoid defaulting on 96 hours. CMS, TJC and Texas state law allow covering physicians to authenticate a colleague’s orders.

Nursing staff place a pink post-it reminder flag, to identify verbal/telephone orders awaiting authentication. Please review and address these reminder flags when you round on your patients. Fall Physician Satisfaction Survey Deadline Extension

The deadline for physicians to complete the 2015 Fall Physician Satisfaction Survey online has been extended through Wednesday, November 18, and we need your help to share the news with the physicians at your facilities.

Physician feedback is critical to us in understanding how to make improvements. When we know what is important to our medical staff, we can make changes that lead to a better environment for patient care.

Please help us to promote the survey with physicians throughout your organization by sharing the extended deadline with them. As a reminder, physicians can take the survey online and the results will be submitted immediately.

We've set a target of at least 50 percent participation in the Fall Physician Survey and many of our facilities have already exceed the goal.

We greatly value the opinions and expertise of our medical staff. Thank you in advance for helping us spread the word about the extended online deadline.

News from the Blood Bank
Each unit of blood product transfused in a patient has both the potential to benefit and to harm. Single unit red blood cell transfusions are encouraged for hemodynamically stable, non-bleeding, hospitalized patients. Additional units in these patients should only be prescribed after re-assessment of the patient, their vital signs and their hemoglobin value.

In clinically stable, non-bleeding patients, single unit platelet transfusions are usually clinically sufficient to achieve hemostasis as one dose will result in an increase of up to 30,000/dl.

In order to promote safe and effective blood product transfusions, blood product orders that meet the following criteria will be reviewed prospectively.
  • RBC unit orders for patients with a pre-transfusion hemoglobin greater than 7 g/dl and no further documented clinical indication.
  • RBC unit orders for patients with a pre-transfusion hemoglobin greater than 8 g/dl.
  • Requests for greater than 1 unit of RBCs or platelets in the absence of active, hemodynamically unstable bleeding.

New Scored Gram Stain for Bacterial Vaginosis

The scored Gram stain is the gold standard for the diagnosis of bacterial vaginosis. Baptist Health System microbiology laboratory now performs this test in conjunction with examination of a wet prep. When clue cells are identified on the wet prep, a scored Gram stain will be performed. The Gram stain is examined for the presence or absence of various bacterial forms and a score is generated. This score is translated into one of three interpretations: normal vaginal flora, consistent with bacterial vaginosis, or intermediate. An intermediate score may be seen with other pathogens and in early bacterial vaginosis, and clinical correlation is required.

Instructions for obtaining Physician CME / Completion Credit

Use of Rubber Signature Stamp - What You Need to Know

As a reminder the Baptist Health System does not accept orders that are authenticated ONLY with a rubber signature stamp.   The Medical Staff Rules and Regulations include the following information under the Section 7.07 Authentication:

"The use of a rubber stamp with the physician's name to facilitate legibility and prompt the inclusion of a time and date is encouraged, however all signatures in the chart must be original.  CMS will not accept stamped signatures.  The physician's original signature must accompany an order.  There shall be no delegation of stamps ..."

Therefore, if you use a rubber stamp your original signature must also be included on the order.  Your rubber stamp cannot be delegated for use by other staff in your practice.   If you have any questions or need additional information please contact the BHS Chief Quality Officer, Emily Volk M.D. at 210- 297-1265.

We are pleased to announce that the Cerner Millennium electronic health record will go-live in the Baptist Health System in the late 3rd quarter to early 4th quarter of 2016.
 
Earlier this year we sent out a memo to the BHS Medical Staff notifying you of a revision in the IMPACT plan and timeline with an estimated adoption date for Cerner some time possibly in early 2016. The revision was due to an identified need for additional, significant project planning centered around the patient accounting/revenue cycle systems in order to ensure the best possible outcomes for our patients, physicians, staff and the BHS hospitals. That effort has been completed to the point we can announce the new go-live window.
 
The BHS project team, including physician leaders, has been working diligently to implement the IMPACT transition. This will result in a huge change for almost every electronic system and workflow within Baptist Health System. We appreciate the energy and engagement that physicians, leaders and staff have put into this project thus far.
 
As a reminder, the Tenet/Cerner team we're working with has implemented the IMPACT transition in 62 other Tenet hospitals to date with great success. However, during the course of our planning sessions, it has become clear that our large and complex group of hospitals will require a different approach in order to be successful in this comprehensive upgrade. (To put our size into perspective, keep in mind that our five hospitals are licensed through CMS as one hospital with more than 1,700 beds. This makes us the second largest hospital in the country, as far as CMS is concerned.)
 
We are absolutely committed to this transition, and we will continue to keep you informed especially as the definitive hospital go-live dates become available early next year. We thank you for your full participation in this project, both in adoption and with post go-live sustainment as a valued member of the Baptist Health System Medical Staff.
